NURS 453: Clinical Management: Adult Health II


Purpose of the Assignment: The purpose of this case study it to integrate knowledge from the
humanities and sciences, including nursing research and theory, to plan, provide, and evaluate
holistic care provided to a client selected during this clinical rotation.


Student Approach to Assignment: My approach to this assignment was to understand the
physiological process of my patient’s condition and utilize elements of the nursing process, such
as assessment, nursing diagnoses, desired outcomes, interventions, and evaluation, to establish a
care plan that holistically cares for my patient and promotes the best patient outcome.


Reasons for Inclusion of Assignment in Portfolio: I have included my Adult Health II case
study in my portfolio because it demonstrates my ability to integrate nursing knowledge,
patient-centered care, and quality and safety principles into practice. It highlights how I use
evidence-based practice and interdisciplinary collaboration to provide comprehensive care.

 

  •  Domains 1,2, 3,: Patient-Centered Care

    •  (7) Demonstrates clinical reasoning in making clinical or professional judgments.

      • ​ In my case study, I used clinical reasoning to make decisions that directly
        impacted my patient’s care. When I first assessed my patient, I carefully
        reviewed their medical history, symptoms, and lab results to identify
        potential complications before they became serious problems. For
        example, I noticed signs that my patient was at risk for electrolyte
        imbalances and acute kidney injury, so I made sure to monitor their fluid
        intake, urine output, and lab values closely. This allowed me to detect
        changes early and work with the healthcare team to make necessary
        adjustments. Additionally, I considered how my patient’s overall condition
        and risk factors might impact their recovery. I adjusted interventions based
        on how they responded to treatment, ensuring that I was providing the best
        possible care. By staying proactive and using my knowledge of
        pathophysiology, I was able to implement strategies that helped improve
        my patient’s stability and prevent complications. This experience
        reinforced the importance of critical thinking and adaptability in nursing practice.​

    •  (10) Demonstrates an awareness of cultural preferences and complementary modalities and their usefulness in promoting health.

      • ​Within my case study, I made sure my patient and their family felt
        respected and involved in their care by using clear, simple language and avoiding medical jargon. Taking the time to listen to their concerns helped
        build trust and made them feel more comfortable. I also noticed that my
        patients felt more at ease when family members were present, so I
        encouraged their involvement in care. Offering emotional support,
        reassurance, and active listening made a big difference in their experience.
        This showed me that nursing isn’t just about treating the illness—it’s about caring for the whole person.​​

  • Domains 2, 3, 5: Patient-Centered Care, Population Health, Quality and Safety

    •  (11) Performs therapeutic interventions that incorporate principles of quality
      management and proper safety techniques.

      • Within my paper, since my patients were at risk for complications, I took
        extra precautions to keep them safe. I performed frequent neuro checks,
        ensured proper positioning, and followed strict fall precautions to prevent
        any setbacks. I also made sure their environment was clear of hazards and
        took time to educate both the patient and their family on how to stay safe
        at home. By being proactive and thorough, I was able to help prevent
        complications and promote a smoother recovery. This experience
        reinforced how small, careful interventions can make a huge difference in a patient’s outcome.

  • Domains 5: Quality and Safety

    • (16) Identifies sources and applications of national safety and quality standards
      and hospital standards of practice to guide nursing practice.

      • Following my paper, to ensured my patient received the best possible care.
        I followed hospital protocols and national safety guidelines at every step. I
        used evidence-based practices for rhabdomyolysis management, including
        hydration strategies and electrolyte monitoring, to prevent complications. I
        also made sure to follow infection control protocols and double-check
        medication administration for accuracy. By staying vigilant and following
        established safety measures, I helped create a safe and effective care
        environment for my patients. This experience reinforced how crucial it is to always prioritize quality and safety in nursing practice.

  • Domains 7: Systems-Based Practice

    •  (34) Demonstrates evidence of participation in Interprofessional Education (IPE)
      activities.

      • In my paper, working with a multidisciplinary team was essential in
        managing my patient’s condition. I collaborated with physicians, physical
        therapists, and dietitians to ensure that every aspect of my patient’s care
        was addressed. For example, since my patient had rhabdomyolysis, I
        worked closely with the physical therapy team to develop a safe
        mobilization plan that would prevent further muscle breakdown while promoting recovery. Additionally, I frequently consulted with the dietitian
        to ensure my patient received appropriate nutritional support, considering
        his risk for electrolyte imbalances and acute kidney injury. By
        coordinating with different disciplines, I helped create a well-rounded care
        plan that prioritized both immediate recovery and long-term health
        management. This experience highlighted the importance of team-based
        decision-making and effective communication in improving patient
        outcomes. Through active participation in interprofessional collaboration,
        I learned how essential it is to leverage the expertise of various healthcare professionals to provide comprehensive and patient-centered care.

  • Domains 8: Informatics and Healthcare Technologies

    • (38) Uses informatics to enhance one’s own knowledge base to provide
      patient-centered care and support teaching.

      • In my paper, I discuss how to manage my patients' care. I utilized
        electronic health records (EHRs) to track lab results, monitor vital signs,
        and document patient progress. Additionally, I accessed evidence-based
        databases to stay informed about best practices for rhabdomyolysis
        treatment. Using informatics tools helped me provide timely, data-driveninterventions that improved patient safety and care effectiveness.
